PLESD is committed to protecting the privacy and confidentiality of student health information. Health information is handled carefully and shared only as permitted or required by law and district procedures.
Protecting Health Information
PLESD respects the privacy of student and family health information. Health records and medical information provided to the District are handled confidentially and used to support the health, safety, and educational needs of students.
Authorization to Release Health Information
There may be times when families need PLESD to share or obtain health information with a healthcare provider or other authorized individual or organization. Written authorization may be required before protected health information can be released.
Notice of Privacy Practices
The Notice of Privacy Practices explains how certain health information may be used or disclosed and describes privacy rights related to that information. Families are encouraged to review the notice and contact the District with any questions.
