Safe Medication Support during the School Day
PLESD works with families and healthcare providers to ensure medications needed during the school day are administered safely and appropriately. Families must provide the required authorization and follow district procedures before medication can be given at school.
How to Provide Medication at School
If your child needs medication during the school day, please complete the following steps before medication can be administered at school.
Step 1 — Obtain the Medication Authorization Form

Step 2 — Complete the Required Authorization
The form must be completed and signed by the parent/guardian and your child's authorized healthcare provider.

Step 3 — Bring the Medication to School
An adult must bring the medication to the school office in its original, properly labeled container.
