| | To register a child for school, parents must present the child's written immunization record from a physician, nurse, or health department. Prior to school entry, children must be immunized for: - Polio
- Diphtheria, Tetanus, and Pertussis
- Measles, Mumps, Rubella (MMR)
- Hepatitis B
- Varicella
A waiver of the immunization requirement may be obtained for religious or health reasons.
